People who want to eat healthy by choosing whole grain foods aren’t helped by product labels that can confuse and mislead consumers, a new study shows.
Almost half were unable to identify the healthier whole-grain option when asked to rely on food package labels, researchers discovered.
A similar proportion of participants were unable to accurately state the whole grain content of different products, according to the study.
Mostly refined grains
Terms like “multigrain”, “contains whole grains”, “honey wheat” and “12-grain” can be used to hawk breads, cereals and crackers as healthier options even if the product mostly contains refined flour, explained lead researcher Parke Wilde, a professor at Tufts University’s School of Nutrition Science and Policy in Boston.
“If they say it contains whole grains, it really does have to contain some whole grains. They would get into trouble if they made a claim that was outright false,” Wilde said. “But it’s totally permitted to say it contains whole grains even if it’s mostly refined grains.
“For terms like multigrain or seven-grain or 12-grain, or the colouring, there are no rules at all,” Wilde continued. “There are no rules against using any of those terms on a refined grain product, or colouring the product brown, which consumers associate with whole grains.”
Current US dietary guidelines recommend that whole grains make up at least half of a person’s overall grain intake. Studies have shown that whole grains can protect against heart disease, type 2 diabetes and cancer.
Refined grains have been ground into flour or meal, with the healthy outer layers of grain stripped away. Whole wheat products contain the entire grain, which boosts the fibre content and adds higher levels of nutrients.
Flashy products
Wilde and his colleagues used two different methods to test consumer savvy when it comes to picking whole grain products.
In one experiment, they asked people to choose between two hypothetical products – one product that contained a lot of whole grains but made no claims on the front of the package, and another that had lower overall whole grains but bore packaging selling itself as “made with whole grains” or “multigrain” or “wheat”.
Both products bore an ingredient list and a Nutrition Facts panel that clearly showed that the less flashy product contained more whole grains, but between 29% and 47% of participants still chose the less-healthy option that marketed itself as a whole grain powerhouse, researchers found.
“The question distinguished people who rely on the whole grain claim on the front of the package, compared to people who look on the ingredients list,” Wilde said. “If you looked at the ingredients list, you would have been able to see which product really contained whole grains.”
The second experiment asked participants to look at four actual grain products and guess whether each contained all whole grain, mostly whole grain, or little to none.
Deceptive claims
Between 43% and 51% of people overstated the whole grain content of the products, based on what the packaging told them, researchers found.
The findings were published on 10 August in the Public Health Nutrition journal.
The study provides strong evidence that could support any attempt by the government to regulate misleading whole grain labels, Wilde said.
“I would say when it comes to deceptive labels, ‘whole grain’ claims are among the worst,” said co-researcher Jennifer Pomeranz, an assistant professor of public health policy and management at the NYU School of Global Public Health.
“Even people with advanced degrees cannot figure out how much whole grain is in these products,” she said in a Tufts news release.
Be on the alert for some words
In the meantime, people will need to do a bit more reading in the supermarket if they want to choose products that truly have more whole grains, Wilde said.
“Read the ingredients list, and know which things are indicators of whole grains,” Wilde said. Look for words like “whole grain” or “whole wheat”, and be on the alert for words like “enriched flour” and “wheat flour” that do not describe whole grains.
“You need to know they list it in decreasing order of weight. If a whole-grain product lists whole grain as its first ingredient, that’s a stronger indication of whole-grain content,” Wilde continued. “That’s something that consumers can do already, but you can see that it would be easier if the label had something like a percentage of whole grain content on the front.”
The Nutrition Facts label can also help people sort out the healthier option between whole grain products, said Dr Maria Pena, who directs endocrine services at Mount Sinai Doctors Forest Hills in New York City.
“Really, the trick is learning how to read a nutrition label. If you don’t know how to read a nutrition label, that’s where you run into trouble,” said Pena, who wasn’t part of the study. “You really have to focus on the amount of carbs the bread has and the amount of fibre. That’s really what helps you determine which bread is better than the other one.
“If a slice of bread has 30 or more carbs, I consider it to be not a healthy choice,” Pena said.

A house where a man killed his mother, sibling and left two other siblings hospitalised.

A man accused of stabbing his mother and sister to death and injuring his two other siblings before attempting to take his own life has appeared in court.
The accused, aged 22, appeared in the Sebokeng Magistrate’s Court on Monday facing charges of murder and attempted murder.
On 29 November last year, he allegedly stabbed his 43-year-old mother and four-year-old sister to death with a knife at their home in Sebokeng.
His 19-year-old brother and nine-year-old sister were also stabbed, but survived the ordeal.

It is alleged the 19-year-old was awoken by his mother’s screams when the accused allegedly attacking her. He was also then allegedly attacked.
He managed to flee to his uncle’s home who lives nearby.
The accused’s uncle then called the police and emergency services.
The case was postponed to 25 September for a High Court date.

British American Tobacco South Africa has urged government to ratify the World Health Organisation’s Illicit Trade Protocol now that the ban on cigarette sales has been lifted, in a bid to combat illicit trade it fears has taken root under the ban.
In a statement issued on Monday, BATSA noted South Africa had signed the comprehensive international rules.
Batsa, which previously held the lion’s share of market share in the country, said it had lost a significant amount of market share under the sales ban, yet research had shown that most smokers had been able to purchase cigarettes.
Illicit traders were cashing in, it said.
Batsa also charged in its statement that cigarette companies associated with the Fair-Trade Independent Tobacco Association took over the tobacco market over the past five months, when the trade of tobacco products was banned – something FITA, for its part, strongly denied.
The ban on cigarettes has been a bone of contention for the tobacco industry, with FITA contesting the ban in court. The Supreme Court of Appeal in Bloemfontein granted FITA leave to appeal in its case against the tobacco ban after the North Gauteng High Court in Pretoria dismissed the appeal. FITA has said it plans to continue with its court action despite the lifting of the ban.
BATSA argued that ratifying the World Health Organisation’s Illicit Trade Protocol would help eradicate the trade of illicit cigarettes and level the playing field for all market competitors by ensuring all cigarette producers are compliant.
“BATSA made its call as the company confirmed comprehensive independent research that shows cigarette brands mainly associated with the FITA have completely taken over the market over the past five months of the sales ban,“ the statement said.
BATSA said the University of Cape Town’s Research Unit on the Economics of Excisable Products found that from 23 000 respondents, 93% of all smokers were still able to purchase cigarettes during the lockdown and tobacco ban.
Professor Corné van Walbeek, who conducted the UCT research, found that BATSA took the largest knock due to the tobacco ban, but found that some retail outlets improperly sold properly obtained and taxed cigarettes out of their own stores during the lockdown.
Fin24 previously reported on the research, which found the market was in “disarray”.
“The market share of BATSA products has dropped from 48.0% prior to lockdown to 8.7% in June. The relatively minuscule amount of people who have reported being able to buy BATSA brands almost certainly obtained them out of pre-lockdown shipped retail stock,” the BATSA statement said.
‘Shenanigans’
FITA chairperson Sinenhlanhla Mnguni said the association rejected BATSA’s accusations with contempt, calling them “another attempt to deflect attention away from its well-documented shenanigans” and to direct law enforcement agencies towards its competitors.
“We also reject any assertions that FITA members have in any way benefited from the ban in the sale of cigarettes during the lockdown period.
“All our members’ exports during the period under the lockdown period within which exports were permitted were at all times strictly supervised and controlled by officials of the South African Revenue Service,” Mnguni said.
However, Mnguni did agree it was critical to maintain a fair market.
Mnguni said it was incumbent on the state to level the playing field in the tobacco industry to give local employers and contributors to the economy a competitive and fair trading environment.
Meanwhile, Philip Morris South Africa managing director Marcelo Nico said in a statement that the company would reopen its 24 IQOS retail stores and resupply trade partners to service IQOS users and smokers looking for alternatives to continued smoking.
“Many of our IQOS consumers were disappointed at not being able to purchase our products during the lockdown ban. We are working to support them and win back their trust. We have implemented strict safety, social distancing and hygiene measures at our stores to protect our consumers and staff,” said Nico.
Nico said regulators should support scientific evidence and improved regulations to benefit smokers and public health.
Another bombshell hit Cricket South Africa (CSA) on Monday as acting chief executive officer Jacques Faul joined Chris Nenzani in resigning from his post.
Faul, who took up the position in December 2019 following the suspension of Thabang Moroe was set to remain in the post until early September but has decided to leave his position early.
Earlier on Monday, in a statement released to the media, CSA announced that Nenzani resigned from his position as president of the organisation.
“Mr. Nenzani led Cricket South Africa with dedication and astutely since 2013. Mr. Nenzani has provided valuable leadership, insight, assistance and direction in advancing the game of cricket with a focus on achieving transformation and access for the majority of the South African population,” the statement read.
“On behalf of Cricket South Africa’s members council and the board of directors, we thank Mr. Nenzani for his contributions to Cricket South Africa and for being a loyal servant to the game of cricket.
As of yet, there has been no comment from the organisation on Faul’s resignation.
Sport24 understands that Cricket South Africa were in the process of holding an emergency board meeting on Monday evening to plot the way forward following the resignations.
