Health24.com | Coronavirus pandemic puts healthcare workers’ mental health on the line
Doctors and nurses are trained to deal with life-and-death situations, to be calm in the face of crisis. But whether it’s in hard-hit New York or places where Covid-19 has yet to surge, medical workers say the pandemic is straining their mental health like nothing before.
“The stress is probably 100 times what you could have imagined it was in the past,” said Judy Davidson, a nurse scientist at University of California, San Diego Health. Davidson, whose research shows nurses were at higher risk for suicide than the general population even before the coronavirus struck, said delivering psychological support to health care workers will be as crucial as providing protective gear.
Stories from some of those on the front lines make clear why.
“To me, it’s almost like going into a battlefield, where every enemy is a sniper,” said Poppy Strochine, a nurse in Cincinnati. “You don’t see him. You don’t know where it’s going to come from. And you don’t have the supplies that you need to keep yourself safe to actually do the battle.”
‘I’d never forgive myself’
As a wound care specialist, she visits people in home after home, unsure of what they might have been exposed to. Strochine has yet to encounter a case of Covid-19 – that she knows of. But “I’m right up against the patient dealing with the wounds. If they cough or sneeze or even touch me, I’m infected.”
The coronavirus, she said, “basically puts you on an island where you are there by yourself”.
She particularly misses the physical touch of her family. “I really can’t do that. Because at any point in time, you don’t know if you’ve got it. And to pass it along to them – I’d never forgive myself.”
Dr Mitchell Elkind, professor of neurology and epidemiology at Columbia University in New York City, has seen the strain.
Much of it stems from uncertainty about how to fight the illness, said Elkind, president-elect of the American Heart Association. “We are constantly learning and updating our recommendations, and I think that makes people a bit uncomfortable.”
Adding to those unknowns: fears about shortages of protective gear and the potential decisions healthcare workers face about who lives and who dies if vital equipment isn’t available.
Nurses ‘on edge’
Elkind also worries about exposing his family to the virus. He’s careful about changing out of his scrubs at the office and showering as soon as he gets home. But he wonders, “Is that overkill? Is it not enough? Should I be staying in a hotel instead of going home? These are the kinds of decisions that we’re making without very much information at all.”
And he’s concerned about the doctor trainees whose careers have been put on hold as they are pressed into service. “It’s interrupted their lives,” he said. “And hopefully, they don’t end up paying for it with their lives.”
Mirna Eads, a floor nurse at a nursing home in Fort Thomas, Kentucky, is on edge because people around her don’t seem to understand what she and her colleagues are up against. Some visitors refuse to comply when asked to wash their hands or wear a mask. Shoppers scowl when she wears a mask at the grocery store. “I get looks like, ‘Really? You’re taking this awful serious, don’t you think?'”
Nela Hadzic, a New Jersey-based union organiser with the Northeast Nurses Association, said nurses have been stressed by the lack of gear, administrators who won’t communicate openly and, again, those fears of infecting loved ones.
“That’s the stuff that’s making nurses cry,” she said.
Davidson said health care employers urgently need to be paying attention to workers’ mental health. “Organisations need to have psychological support systems in place right now,” she said. “This is not an option. It just has to be.”
Solace in humour
She listed several approaches that have been shown to work. Her facility adapted its Healer Education Assessment and Referral (HEAR) programme, designed as a suicide prevention tool, to focus on workers affected by the pandemic. Code Lavender, a Cleveland Clinic programme, uses trained teams to respond to workers who say they’re having a mental health emergency. A University of Missouri program uses “emotional first aiders”, to look out for people who might be having a bad day. And Ohio State University’s MINDBODYSTRONG offers a series of classes that build resilience.
Workers need to be watching out for one another, too, and take action. Know the number of your employee assistance programme and keep the number of The South African Depression and Anxiety Group (SADAG) handy.
“If you’re with a co-worker that you have identified as high-risk, stay with them through that moment,” Davidson suggested. “Do not leave them. Tell them that you recognise they need help, you want them to get help, and you’re not going to leave them.”
Strochine, the wound care nurse, finds solace in humour, and in getting out to the creek near her house. “I’ll sit beside the creek for hours on end and just meditate and relax. And that seems to help.”
She and Elkind both said people can support medical workers by doing something simple: staying home.
“It’s so hard to conceive of how staying at home is a benefit, because you feel like you’re not doing anything,” Elkind said. “People want to get out there and do something positive. They don’t want to stay back and not do something. But in this case, the paradox is that staying home and staying away from people really is of great benefit.”

Nienaber, previously the team’s defence guru, has taken over the head coaching responsibilities from Rassie Erasmus, who has taken a back seat to focus on his director or rugby role at SA Rugby.
In an interview with Netwerk24, Etzebeth said he has no doubt that all the players from last year’s victorious World Cup squad would have picked Nienaber as the new coach.
“Jacques is my coach. So any player will have to be careful what they answer!,” Etzebeth chuckled, before adding: “But I can honestly say that all 31 guys in that group would have mentioned Jacques’ name if they were asked who the next Bok coach should be,” Etzebeth said.
Erasmus and Nienaber have a long history, having first met in the army before crossing paths at the University of the Free State’s Shimlas team where Nienaber was the physiotherapist and Erasmus the captain.
When Erasmus became coach of the Free State Cheetahs, he appointed Nienaber as strength and conditioning coach, with the latter later shifting his focus to the team’s defensive structures.
Nienaber followed Erasmus to the Stormers and also Munster in Ireland, before starting at the Springboks in 2018.
“He worked under Rassie for long and knows exactly what it takes to be a head coach. Ever since I started working with Jacques at the Stormers in 2012, it was unbelievable for me how he got the best out of players. His defence strategies are at another level,” Etzebeth added.
Etzebeth, who currently plies his trade at French club Toulon, is back in South Africa as rugby takes a hiatus due to the coronavirus outbreak.
The burly lock said in an interview over the weekend that he was “grateful” to his French club for allowing him to return home before South Africa went into a 21-day lockdown.
The 28-year-old has been on Toulon’s books since last year, having played for the Stormers between 2012 and 2019.
The World Cup winner has played 85 Tests since making his international debut against England in Durban in 2012.

The virus has now killed more than 86,000 people and infected 1.5 million, sparing almost no country and tipping the world into a devastating economic crisis as global commerce shudders to a halt.
For the second straight day, the US grieved nearly 2,000 deaths on Wednesday, as flags flew at half-mast in hardest-hit New York.
There was also a record death toll of 938 over 24 hours in Britain, where Prime Minister Boris Johnson spent a third night in intensive care, his condition said to be “improving.”
France saw its total number of dead climb over 10,000 as the country prepared to extend its strict lockdown measures.
Spain and Italy are still seeing hundreds of deaths per day despite tentative signs the disease may have peaked.
And the pandemic is marching into areas previously only lightly affected: in Africa, Ethiopia declared a state of emergency and Liberia said it was locking down its capital Monrovia.

‘I don’t give a damn’ – WHO chief addresses death threats, racist insults
Director general of the World Health Organisation (WHO) , Dr Tedros Adhanom Ghebreyesus, says he has been receiving death threats and racist insults from people.He said some personal attacks came from people in Taiwan a few months ago.
Ghebreyesus brushed off these personal attacks on Wednesday in a press conference, but said a line was crossed when people insulted Africa. He would have been alluding to rumours being circulated about vaccines being tested in Africa.
“Then I don’t tolerate [it], then I say people are crossing a line…we cannot tolerate that,” he said. “When it’s personal, when it’s death threats, I didn’t care, I didn’t respond… I am a very proud black person.
“I assure you, we [as WHO] will do everything we can to serve humanity. We will do everything to help us have no regrets at all.”

As of May, Sassa grants for disability and Older Persons Grants will be paid from the 4th day of the month. All other grants will be paid from the 6th of every month.
Grant collection days have changed since the national lockdown.
The body has said that the grant payment for April was brought forward to the 30 March and the first two days were dedicated to people with disabilities and older persons to ensure compliance with new regulations.
However, child support grant beneficiaries were seen to be impatient, Sassa said, and could not wait for the 1 April and descended upon payment channels as early as 30 March.
“This posed all sorts of health challenges and it became difficult at some stores and ATMs to maintain social distancing,” a statement read, citing a “nasty scene” in a Dobsonville shopping centre in Soweto where a group of youngsters were captured on video pushing older people in order to access payment infrastructure first.
As a result, the above changes have taken effect.

President Cyril Ramaphosa will make “serious decisions” about the lockdown after considering all options based on scientific modelling and advice as well as the possible economic repercussions of the nationwide shutdown.
Jackson Mthembu, the minister in the presidency, told News24 Ramaphosa had not yet taken a decision whether the current lockdown would be extended or whether a second lockdown was being considered.
There have been no proposals tabled at the National Command Council which was the government body chaired by Ramaphosa tasked with managing South Africa’s response to the coronavirus, he said.
Three health experts say any premature suspension of the lockdown will lead to an increase in the rate of infections, which seemingly have started to slow down.

Economists, however, are warning a prolonged lockdown may cause irreparable harm to the economy, with Finance Minister Tito Mboweni set to hold a press conference on Thursday to address recent economic developments, which include another ratings downgrade.

According to Mthembu, the president would look to consult with various sectors and stakeholders before making a decision as he did before he announced the lockdown almost three weeks ago.
“First, we must make a proper assessment if we have achieved all of our objectives, which was to a greater degree to flatten the infection curve and ensure it doesn’t peak. What we’ll have to review is whether we’ve achieved that,” he said, adding the National Institute of Communicable Diseases (NICD) and the Department of Health were leading this assessment.
“The issue of the [lockdown] extension has not yet arisen. If it comes, there will be serious decisions to be made. The National Command Council discusses all recommendations that come before it, and there must be scientific evidence supporting any recommendation from the national joint operational and intelligence structure,” Mthembu said.
Ramaphosa will consult again
Various stakeholders, both inside and outside the government, are currently preparing models attempting to predict the rate of infection as well as to determine whether the current lockdown has been successful. These models, including those being developed outside of the government, will be presented to the National Command Centre.
The economic implications of the lockdown, as well as a possible second lockdown, is being debated at the highest levels of the state, with officials and ministers looking at ways to kick-start the economy as soon as possible.
The SA Reserve Bank expects GDP to contract by between 2 and 4% after the lockdown.
A senior academic, who is part of one of several groups working on the modelling and projection of the Covid-19 outbreak, said the rate of new infections had now reached a linear trajectory, seemingly showing the lockdown was working.
He did not wish to be quoted as he is not authorised to speak to the media.

Compared with in the rate in mid-late March when the increase in infection rates was “exponential”, infection rates have slowed significantly.
But this was a snapshot in time and did not factor in the lifting of the lockdown or certain conditions of it, he said. If the lockdown was to be lifted, the rate of infections would again increase drastically.
Another epidemiologist, who is also involved in efforts to provide information to the government and also not authorised to speak to the media, said the peak infection rate would only be reached in a few months and emphasised that testing efforts, described as currently inadequate, would need to be ramped up significantly.
Limiting transmissions
Dr Kerrin Begg, a public health specialist at the University of Stellenbosch, said from a public health point of view, the best way to limit community transmissions was to limit movement dramatically.
The flattened peak of infections being observed now was mostly based on imported cases, she said.
“It is likely that our big curve of the epidemic is only starting now. So, returning to a fully operational society has significant risk in that as you expose people to each other, cases could really increase,” added Begg.
Another concern was experts were realising the asymptomatic spread of the disease had been higher than initially thought – the lockdown helped to minimise this, she said.
There were other ways to minimise gathering outside of a full-blown lockdown, Begg added, including deferring the opening of schools and universities, limiting the number of people allowed to gather to much lower than 100, and continuing to encourage hand washing and other hygiene practices.
The government could also consider other options relating to densely populated areas, such as restricting movement to not just the home, but a smaller area or block, which would still limit the spread of the disease.
“We know from other pandemics that any restriction of movement helps … while it is difficult, restricting movement is the most effective way to limit the spread of the disease.”
