Health24.com | Paralysed patients undergo complex ‘nerve transfer’ surgery – and regain movement in hands and arms
Thirteen paralysed young adults have regained elbow and hand movement after undergoing complex surgery in Australia, surgeons report.
The patients now brush their hair and teeth, feed themselves and put on makeup – tasks that were impossible before the “nerve transfer” surgery, the doctors report in The Lancet medical journal.
Cost-effective procedure
“For people with tetraplegia [paralysis of upper and lower limbs], improvement in hand function is the single most important goal,” said study leader Dr Natasha van Zyl, of Austin Health in Melbourne.
“Stem cells and neuroprostheses could change the landscape of regenerative medicine in the future,” noted Dr Ida Fox of Washington University in St. Louis.
But, “for now, nerve transfers are a cost-effective way to harness the body’s innate capability to restore movement in a paralysed limb,” said Fox in an editorial accompanying the new study.
The patients underwent extensive physical therapy after the procedure. Two years out, they are using electronic devices and tools, and handling money and credit cards, according to the report.
The restored elbow movement improved their ability to move their wheelchair and to get into bed or a car, the authors said.
Single or multiple transfers
The study included 16 young adults (average age 27) with paralysis of the upper and lower limbs due to spinal cord injuries suffered within the previous 18 months. Most of the injuries occurred in sports or motor vehicle crashes.
The patients underwent single or multiple nerve transfers in one or both arms. Surgeons took nerves from functioning expendable muscles above the spinal injury and attached them to paralysed muscles below the spinal injury.
Altogether, 59 nerve transfers were completed in 13 men and three women and a total of 27 limbs. In 10 patients, nerve transfers were combined with tendon transfers to improve hand function.
The patients were assessed before surgery, and one and two years afterward. Two patients did not complete the follow-up, and one died of causes unrelated to the surgery.
At the two-year assessment, the patients showed major improvements in the ability to pick up and release objects.
The results suggest that nerve transfers can provide improvements similar to traditional tendon transfers, but with smaller incisions and shorter immobilisation times after surgery, according to the researchers.
Excellent results
One hand surgeon who wasn’t involved in the trial said these types of operations have been used for years in other contexts.
“Nerve transfers have been used for the past decade for multiple conditions with excellent results,” explained Dr Steven Beldner. He helps direct the New York Hand and Wrist Center at Lenox Hill Hospital in New York City.
This type of surgery appears to work when spinal cord injuries are involved, as well, Bender said. As he explained, “most muscles have multiple nerve branches controlling the same muscle function. [In these surgeries] some of these muscle branches are transferred to other muscles, which have lost their voluntary control due to nerve injury.”
However, the Australian team stressed that the operation isn’t foolproof. Four nerve transfers failed in three patients, and the researchers said further study is needed to determine which patients are the best candidates for nerve transfer surgery.
“We believe that nerve transfer surgery offers an exciting new option, offering individuals with paralysis the possibility of regaining arm and hand functions to perform everyday tasks, and giving them greater independence and the ability to participate more easily in family and work life,” van Zyl said in a journal news release.
“What’s more, we have shown that nerve transfers can be successfully combined with traditional tendon transfer techniques to maximise benefits,” van Zyl said.

Cape Town – Springbok lock Lood de Jager looks set to return to action after a long injury layoff.
De Jager, who was named Bulls captain ahead of the 2019 Super Rugby campaign, has been sidelined since March with a shoulder injury but Blue Bulls team doctor Herman Rossouw said the second-rower could be included for Saturday’s Currie Cup opener against Western Province at Newlands.
“Lood has recovered fully from his shoulder injury and it looks like there is a chance that he will get some game time before the Springboks will need him,” Rossouw said on Monday.
Elsewhere, flyhalf Marnitz Boshoff has fully recovered from his knee injury and will likely wear the No 10 jersey against Western Province, with Handre Pollard on Springbok duty.
Former Lions lock Andries Ferreira, whose move to the Hurricanes failed due to injury, has linked up with the Blue Bulls and could be considered this weekend.
There was also some bad news as Rossouw confirmed that flank Nic de Jager tore his Achilles tendon in a warm-up match against the Pumas and will sit out the entire Currie Cup.
Saturday’s clash at Newlands is scheduled for 17:15.

Micheal Nkosi is convinced that the next crop of international tennis superstars will come from Yeoville in central Johannesburg.
The 64-year-old, who has been playing the sport for more than three decades, has offered free tennis lessons at the Yeoville Tennis Club since 2003.
The beginning
He held his first tennis racquet in the early 1980s at the Arthur Ashe Tennis Centre in Soweto – it was love at first serve.
However, it quickly became apparent that he did not have the competitive edge to win big tennis tournaments.
“I realised I was very less skilled in the techniques,” Nkosi told News24.
He could not afford to pay for private lessons so he looked for other avenues that would allow him to stay on the court.
The next best thing was coaching.
“I found that people are so engrossed in playing tennis themselves and they forget about developing other people.”
Fortunately, in 1996, Nkosi was able to attend a coaching seminar after winning a scholarship sponsored by Gauteng’s sports department.
The year-long seminar was facilitated by Tennis South Africa. The rest is history.
Keeping children safe
Now Nkosi offers lessons five times a week. He has more than 50 students on rotation.
For him, it is more than about teaching the children how to volley; it is about creating upstanding citizens.
“I consider myself a mentor and a parent to these kids. I teach them discipline, friendship and teamwork,” he said.
He also warns them about the pitfalls of drug abuse and gangsterism.
Nkosi believes that tennis improves his students’ physical and mental prowess.
“Tennis is a big mind game. It’s a lot of science and mathematics involved. Kids are taught to think, it’s almost the same as chess.”
Next stop: Wimbledon
He is determined to mould his students into world class athletes.
“One way or another I will be sitting at Wimbledon with a player that comes from Yeoville,” he said.
He regularly enters his students in tournaments, and offers lessons on weekends.
According to Nkosi, there is no such thing as too much practise.
News24.com | Gang Wars Part 3: One month, 883 gunshots – Hanover Park’s gunfire in numbers
This story is part of a three-part “Gang Wars” series, which this week focuses on Hanover Park.
Since the start of the year, 45 people have been murdered in Hanover Park in the Western Cape and 12 people lost their lives to violence in June, the bloodiest month in the gang-ravaged area this year.
Escalated gang violence erupted in the Cape Flats suburb in April. Eight people were killed that month, eight in May and 12 in June.
According to sources, the recent escalation in violence is due to a turf war between the Americans and the Ghetto Kids, further fuelled by retaliation killings.
They are two of the biggest and most dangerous gangs in Hanover Park.
Touted as an enabler to a much bigger strategy in combating gun and gang violence, the City of Cape Town launched the multi-million rand Shot Spotter gunfire detection technology pilot project in Hanover Park five years ago.
In three months, the data showed that 1 106 shots had been fired in 344 recorded incidents.

On 21 of those days, only a single gunshot was recorded. There were seven days where no gunshots were recorded.
The highest number of gunshots fired in a single incident was 25, the data showed. Last July, a staggering 883 gunshots were fired, that equates to around 28 a day.
Most of the shootings recorded occurred between 20:00 and 05:00 and incidents peaked between 23:00 and 03:00, the numbers indicated.
According to Shot Spotter data for 2018, 3 787 bullets were fired in Hanover Park in 1 222 separate incidents.
In the first three months of this year, 1 208 shots were fired in 389 incidents.
According to the provincial Department of Community Safety, analysis of the police’s crime statistics for the last financial year showed that 83% – or 808 – of gang-related murders in the country were recorded in the Western Cape.
In the province’s 26 gang hot-spot precincts, 45% of the 3 729 murders, 56% of the attempted murders and 57% of the gun-related offences were recorded.
The role of illegal firearms
According to advocacy group Gun Free SA, gang violence has become endemic in Cape Town, largely as a result of illegal firearms being distributed to the Cape Flats.
In 2019, nearly 2 000 people have died from gun violence in the last financial year. The public reported the loss or theft of 24 guns every day.
To avoid similar cases and to stop gun violence from escalating even further, the NGO on Global Gun Destruction Day – July 9 – urged police to destroy seized and surrendered firearms.
“It is globally recognised that destroying excess, unwanted and recovered firearms, ammunition and firearm parts is the only way to guarantee that these are not leaked into the illegal pool of weapons,” Gun Free SA’s director Adèle Kirsten said.
Guns have replaced knives as the leading cause of murder in South Africa, the organisation said. Currently, 23 people are shot and killed every day. A marked increase from the 18 a day in 2009.
Where to next for the Anti-Gang Unit?
President Cyril Ramaphosa launched the Anti-Gang Unit in Hanover Park last year.
He said the national anti-gang strategy sought to address gangsterism in a holistic manner with a focus on “street level outreach, community development, conflict mediation and changing community norms in order to reduce violence and criminality”.
In June, Minister of Police Bheki Cele said the unit did not have enough officers and the police were looking at other ways to curb crime in the province.
He eventually escalated it to his national office, amid reported infighting, budgeting and capacity constraints, and a highly publicised botched operation in Philippi last month that saw five Anti-Gang Unit members shot and injured.
Following the incident, the unit’s head, General André Lincoln, in a scathing letter, requested that three officers be removed from the unit due to a “trust breakdown”.
Western Cape police commissioner Lieutenant General Khombinkosi Jula has now also been offered a move back to KwaZulu-Natal amid reports of internal conflict in the provincial offices.

Last week, community activist Colin Arendse claimed the Anti-Gang Unit had not been allocated a sufficient budget.
He added it had to compete with a second unit, the major offences reaction team (MORT), saying police stations and specialised units were denuded when officers were redeployed to it. Cele is probing the claims.
This week, Western Cape Premier Alan Winde confirmed that more than 2 300 people have been murdered in the province between November 2018, when the Anti-Gang Unit was launched, and May this year.

Getting your upper arms in shape is a worthy ambition in any season, and the triceps are the muscles to target for a sleek look.
Toning them can be a challenge, but the “triceps extension” and variations of this key strength training move make up the core of an effective training routine.
Triceps variations
For the basic triceps extension, lie flat on a mat with knees bent and hip-width apart or on a weight bench with feet flat on the floor in front of you. Hold the ends of a dumbbell or the handles of a weighted ball with both hands. Lift arms straight up to the ceiling. From this position, and without moving the upper arms, bend elbows to bring the weight toward your forehead until arms make a 90-degree angle. With control, straighten arms. Repeat.
For the first variation, from the same starting position, hold a dumbbell in each hand. Raise arms straight up to the ceiling, with palms facing away from you. Again, moving only your forearms, bend elbows until upper arms and forearms make a 90-degree angle. With control, straighten arms, but before you lower them again, rotate your wrists so that palms are facing you as you lower your forearms. Straighten arms and continue to reverse the position of your wrists with each rep.
For the second variation, from the same starting position and with a dumbbell in each hand, raise arms straight up to the ceiling. Again, moving only your forearms, bend elbows to lower the weights to the floor on either side of your head. Avoid moving your shoulders. Straighten arms to return to start.
For each exercise, aim for three sets of up to 15 reps each. Start with a weight that allows you to do eight reps per set in good form. Once you can complete 15 reps per set before reaching muscle failure, increase the weight.
