Community Media Trust (CMT) is a South African NGO focusing on communication in the fields of basic human rights. CMT aims to empower individuals and communities, encouraging them to take part in managing and improving their own lives, promoting healthy and health-seeking behaviour and basic human rights. It does this through media and outreach programmes that provide scientific information in a format that is easy to understand.
An exciting opportunity exists for a qualified community referral officer to participate in a ground-breaking HIV prevention programme focusing on counselling and Testing (HTS) in the greater Gauteng area as CMT seeks to employ a community referral officer who will be responsible for strengthening linkages between facilitators and HIV Testing Services (HTS) as well as other health and social services.

Key duties and responsibilities: (may include but are not limited to)
- Strengthen linkages between the multi-session facilitators and HIV Testing Services (HTS) as well as other health and social services.
- Work to ensure that the referral loop is closed and that the referral is completed satisfactorily.
- Work closely with DOH and other stakeholders to establish community-based structures that will create demand for HTS and for treatment, care and support.
- Coordinate activities with identified health facilities to ensure that clients referred from structured programmes are provided with the continuum of care within a health facility.
- Report weekly to the coordinator on tracking and tracing of referrals and completing the referral registers.
- Work closely with the M&E department to ensure that the data is collated, submitted and verified.

Requirements
- Grade 12 or equivalent qualification
- At least five years’ experience working in HIV programmes or a health facility
- Valid code eight driver’s license
- Knowledge of HIV/AIDS, TB and other related health issues required
- Intermediate level computer skills (MS Office)
